COVID-19 Pre-appointment management at Claremont Dental

We look forward to seeing you for your scheduled dental appointment. To keep you, other patients, visitors and our staff safe we ask you to consider the following questions:

• • do you have COVID-19 (we are not approved to see COVID-19 positive patients)
  • do you have COVID-19 symptoms (fever, cough, sore throat, loss of smell or taste)
  • are you awaiting test results for COVID-19
  • have you been in contact with anyone diagnosed or suspected to have COVID-19
  • have you returned from international travel within the last two weeks
  • have been in an area identified as moderate or high risk for community transmission

If you answer YES to any of the above questions, please give our surgery a call on 9383 1200.

Please have your COVID-19 vaccination certificate ready for presentation at reception on arrival. You will then be asked to complete a COVID-19 risk questionnaire.

Please wear a mask to your appointment and attend alone whenever possible.
